STUDENT REGISTRATION AND ADDRESS CHANGESALL REGISTRATIONS/CHANGE OF ADDRESSES WILL BE COMPLETED ONLINE ONLYStudent RegistrationFor Information on New or Re-enrollment Student Registrat ions, click the Student Registration link on the left hand side.Change of AddressParents/Legal Guardians must submit ALL REQUIRED DOCUMENTATION to complete a CHANGE OF ADDRESS.Please email ALL required documents to either nicholasm@hasdk12.org or romerod@hasdk12.org.ADDRESS CHANGE and TRANSFERS between HASD Schools require the following documentsRequired documentation is listed below:
- Parent or Legal Guardian's Photo ID
- Three (3) Proofs of Address
- Examples:
- Internal Revenue Statement
- W2 Form
- State ID Card
- Voter Registration Card
- Property Tax Bill
- Insurance Statement
- Property Deed
- Driver's License
- Vehicle Registration
- Lease
- The following proofs must be current (within 3 months)
- Billing Statement
- Current Pay Stub
- Utility Statement
- Bank Statement
- Cable Statement
- Multiple Occupancy Notarized Letter signed by both parties (if applicable)
- Custodial agreements/court orders/guardianship papers/etc. (if applicable)
ENVELOPES, PROMOTIONAL MAIL, SHIPPING LABELS OR POST OFFICE CHANGE OF ADDRESS FORMS WILL NOT BE ACCEPTED AS A PROOF OF ADDRESS.
Address Changes and Transfers between HASD Schools CANNOT be completed without the required materials.Questions may be directed to the registration department of the HASD at 570-459-3111 extensions 3153 or 3281.
