     ALL REGISTRATIONS/CHANGE OF ADDRESSES WILL BE COMPLETED ONLINE ONLY
     
     
    Student Registration
    For Information on New or Re-enrollment Student Registrat ions, click the Student Registration link on the left hand side.
     
    Change of Address
    Parents/Legal Guardians must submit ALL REQUIRED DOCUMENTATION  to complete a CHANGE OF ADDRESS.
     
    Please email ALL required documents to either nicholasm@hasdk12.org or romerod@hasdk12.org
     
    ADDRESS CHANGE and TRANSFERS between HASD Schools require the following documents
     
    Required documentation is listed below:
     
    1.  Parent or Legal Guardian's Photo ID
    2.  Three (3) Proofs of Address
      •    Examples:
        • Internal Revenue Statement
        • W2 Form
        • State ID Card
        • Voter Registration Card
        • Property Tax Bill
        • Insurance Statement
        • Property Deed
        • Driver's License
        • Vehicle Registration
        • Lease    


      • The following proofs must be current (within 3 months)
        • Billing Statement
        • Current Pay Stub
        • Utility Statement
        • Bank Statement
        • Cable Statement


    3.   Multiple Occupancy Notarized Letter signed by both parties (if applicable)
    4.   Custodial agreements/court orders/guardianship papers/etc. (if applicable)

     

    ENVELOPES, PROMOTIONAL MAIL, SHIPPING LABELS OR POST OFFICE CHANGE OF ADDRESS FORMS WILL NOT BE ACCEPTED AS A PROOF OF ADDRESS. 

     

    Address Changes and Transfers between HASD Schools CANNOT be completed without the required materials.
