• Reevaluations are completed every 2 years for students with an Intellectual Disability and every 3 years for all other disability categories. 

     

    Permission to Re-evaluate/ Re-evaluation Request Form 

    This form is used when a parent makes an ORAL request for reevaluation to any professional school employee or administrator, the parent must receive this form within 10 calendar days of the oral request.  This form provides the means of getting the request in writing.  Be aware that this does not consent to reevaluation.

     

    Permission to Reevaluate- Prior Written Notice For A Reevaluation and Request For Consent Form

    This form is used to provide parents with prior written notice that the school/LEA is proposing to conduct a reevaluation to collect additional data in order to determine educational needs, including whether additions or modifications are needed to enable the student to meet IEP goals and to participate in the general curriculum.  You must have this form checked and signed before you can start any testing.  The team should determine whether additional data is necessary, and if so, of what nature. Once that is determined, the permission to reevaluate gets issued. It is suggested that the permission be issued approximately 90 days prior to the due date of the RR.  You will be alerted to this on your dashboard as well (see below).
